PRE-K students can practice their Kindergarten skills throughout the school year and all summer long at UPSTART. This is a free, technology-delivered kindergarten readiness program that will give your child a fun start in reading, math, and science (with a focus on reading) in just 15 minutes a day, 5 days a week. In order to complete the kindergarten registration process please complete, print, and submit the following information to the Centerville Elementary Front Office. Please make sure all necessary documentation is included.
Your child will not be assigned a teacher or class at Centerville Elementary until the REGISTRATION PROCESS IS COMPLETED.
The following documents MUST be provided at the time of enrollment:
- Student Information Form – This is a fillable form, which means you can type your information into the form and then print it out to bring with you in when you register.
- Guardianship Form – Fill out this form, sign it, and bring it to the office with any legal paperwork you may have regarding custody of the student you are registering.
- Immunization Record – Please provide the office with an original immunization record. If your student has not completed the required immunizations, they must be completed before school starts.Here is a list of required immunizations: Kindergarten Immunization Requirements.
- Birth Certificate - This MUST be the original-sized birth certificate. We will make a copy of it here in the office for our records. You can obtain a copy from your current school or the Health Department.
- Proof of Residency - you MUST provide:
- one document from Column A AND one document from Column B OR
- two documents from Column B
- Parent / Legal Guardian Picture ID - We will use this to verify that you are the person authorized to register your student.
- Homeless Assistance Questionnaire (McKinney-Vento Eligibility) - Answer the two questions on the form.
- Emergency Release Form - Fill out and sign the form in the office.
- I.E.P. - (Resource / Speech / Preschool / Headstart / 504) if applicable
- Kindergarten Student Information Survey -- This will help place your child in the best place, based on his or her needs.
